Skilled Nurse Services (RN/LPN)

Provide direct patient care in the home setting. Works in coordination with patient’s physician to meet individual needs of the patient. Skilled nurse services include disease and medication management with optional weekly medication setup and administration (if applicable), illness assessment and instruction at each visit with measurement of vitals as ordered by your physician, and assessment of risk for hospitalization. Other services include administration of IV therapy, wound care, dressing changes, ostomy/colostomy care, and assistance with obtaining medical supplies and/or equipment. Skilled nurse services also include care provided by a CHHA.
